Fusion Medical Staffing is seeking a skilled MDS Coordinator for a 13-week travel assignment in Bangor, Maine. Required Qualifications

  • One year of recent experience as an MDS Coordinator or equivalent role
  • Valid RN or LPN license in compliance with state regulations
  • Current BLS (AHA/ARC) or CPR certification

Preferred Qualifications

  • ACLS Certification
  • Other certifications may be required depending on facility requirements

Summary

An MDS Coordinator manages the Minimum Data Set (MDS) assessment process in a long-term care (LTC) facility, ensuring assessments are completed accurately and on time. They play a key role in coordinating care plans, making sure they align with both resident needs and regulatory requirements. Working closely with healthcare teams, MDS Coordinators help develop care plans that improve resident outcomes while also supporting proper facility reimbursement.

Essential Work Functions

  • Evaluate the quality of care provided to long-term care residents, ensuring it meets both regulatory requirements and facility standards
  • Conduct initial assessments and interviews with new residents, gathering key information to create personalized care plans
  • Develop and implement care plans with clear short-term and long-term goals to support each resident’s health and well-being
  • Lead care plan meetings with nurses, therapists, caregivers, and families to ensure a collaborative approach to resident care
  • Monitor and follow up on resident concerns, complaints, and incidents, ensuring they are properly documented and addressed
  • Work with specialists and community resources to coordinate additional services that help residents meet their care goals
  • Report findings and make recommendations to management to help improve care quality and facility operations
  • Ensure all MDS assessments and documentation comply with federal and state regulations
  • Support the facility director and administrator by providing expert guidance on resident assessments, care planning, and compliance
  • Perform other duties as assigned within the scope of practice

Required Essential Skills

  • Critical thinking, service excellence, and good interpersonal communication skills
  • The ability to read, write, and communicate in the English language
  • Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills
  • Physical Abilities - Must be able to remain in a stationary position, move about, move equipment (50-100lbs), push, pull, and bend
  • Interpersonal Skills - Must be able to work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalism
  • Technical/Motor Skills - Must have the ability to grasp, perform fine manipulation, push/pull, and move about when assisting with procedures and/or using department equipment
  • Mental Requirements - Must be able to cope with frequent contact with the general public and customers while meeting deadlines under pressure. Must be able to work under close supervision occasionally, as well as working without assistance from other personnel. Must be able to contend with irregular activity schedules occasionally and continuous concentration to detail
  • Sensory - Must possess visual acuity and ability to effectively communicate
